Community Forum Survey

This survey has been created as a way to provide the Gender Centre with quality and accurate feedback regarding the support, events and services we provide to our clients, to help ensure that we can continue to provide a quality service to these clients, and through them improve upon the education we provide to other service providers who may also be involved with clients with gender issues.

If you are a current resident of New South Wales or have previously lived in New South Wales you are eligible to complete this survey. If you are located outside New South Wales and have never lived in this State, The Gender Centre is still interested in your experiences and opinions, although we ask that you provide this information to us by email and not as part of this survey.

The only required fields in this survey are marked as such (  Required ) and while all other questions in this survey are voluntary, we ask that participants submit their completed survey only once to ensure the accuracy of the results. If you have previously completed the same survey as part of a mail-out or while attending the Gender Centre, we ask that you not take part in the online survey.
